Referral form

Waikato DHB and Hauora Waikato clinicians need to use the Ahikāroa form below to refer a tangata whai ora to the service.

The completed form should be emailed to ahikaroa@wisegroup.co.nz.

Ahikāroa referral form

As part of the referral process, we request detailed information about tāngata whai ora, including personal details, health information, information about current supports, any cultural support that may be needed, and a history of clinical care. This information is requested so our teams are well prepared to support people in the best way possible.

It is essential that tāngata whai ora are aware of the information that will be shared and who will have access to this information. Tāngata whai ora must consent to a referral to Ahikāroa and this consent will need to be recorded.

Flexifund form

Clinicians or residential providers can use the Flexifund form below to make a funding request. Requests need to meet the criteria and principles outlined on the Flexifund page.
